The Grace Haven application process consists of 3 easy steps and can be done in person, over the phone, virtually or in the community.
Application Process
Step 1: Call, text or email us and a member of our team will contact you to complete eligibility screening. Click here to contact us.
Step 2: Chat with our intake and review team and infant/parent specialist so we can learn more about you and your child.
Step 3: The intake and review team will determine your eligibility and reach out to you with next steps.
Our programs are available to all accepted day program participants. You will be assigned a case worker and will be supported by our infant/parent specialist and child development team.
Make a Referral
Grace Haven accepts self-referrals, third-party referrals (friends & family), and referrals from all community agencies and programs.
